Convenience Services / S.T.A.R. Service Application

Your application will be processed and sent to you for your original signature and choice of access code. Please note, you must be a MEMBER OF TEXAS BAY AREA CREDIT UNION TO APPLY.

I hereby request authorization for access to my Account(s) at Texas Bay Area Credit Union through the use of S.T.A.R. telephone service. I agree to protect my personal access code and not disclose it to anyone whom I have not authorized to use my Account(s). The Credit Union hereby disclaims any and all liability for any unauthorized use of the S.T.A.R. telephone service.

I agree to the terms and conditions of the DEPOSIT AGREEMENT, TRUTH-IN-SAVINGS RATE AND FEE SCHEDULE, and to any amendment the Credit Union makes from time to time which are incorporated herein. I agree to and acknowledge receipt of a copy of the Deposit Agreement and the Electronic Funds Transfer Agreement. This application is subject to approval by the Credit Union and may be terminated at any time.
